New members can download the application and payment forms by following the instructions included below on this web page. Complete these forms and present them together with payment during surf club office hours.

Renewing members should contact Jo at the office to obtain a copy of their pre-printed membership renewal form. Payment can also be made at the same time.  

The fee schedule for the 2010 - 2011 season is shown below.

Once the season is underway, joining members will pay a prorata fee. Contact the surf club registra on registra@mermaidslsc.org.au for details.

Nipper Family is for the Nipper's and their parents . If any of the parent's children fall into a Senior           
category those children need to be joined into the Senior category not the Nipper Family category.

Refundable if they collect at the Annual collection  - 
if they do not collect it will be treated as a tax deductible donation included in our collection.          
 
All Memberships are subject to approval at the next management committee meeting.
 
The above fees include membership of the Supporter’s Club for all persons aged 18 years and over.
 
Membership includes a secure login access to the club web site and access to the member’s pages pertaining to the category of membership.  
 
Transfers must be approved prior to involvement in activities
 
Family Membership is available for up to 2 adults and all the children that they are jointly, legally and morally responsible for.
 
Probationary: any person for the time period between applying for a membership and the gaining of an award and/or the granting of a formal category of membership
 
Junior Activities: minimum age of five (5) years and up to a maximum age of thirteen (13).
 
Cadet: 13 – 15 years old. A Surf Life Saving Certificate holder which must be updated at an annual proficiency test.
 
Active: Bronze Medallion holder which must be updated at an annual proficiency test. Under 18 classed as Junior
 
Reserve Active: may be applied for by active members who have satisfactorily completed (from the gaining of the Bronze medallion) at least eight (8) years of patrol and club obligations as provided by SLSA and club constitution
 
Long Service:  members who have completed ten (10) years active service or to a member who has completed eight (8) years active service plus four (4) years reserve active service.
 
Award: person who hold a SLSA award of one, or more of the following qualifications – Surf Rescue Certificate, Radio Operators Certificate, resuscitation Certificate, Advanced Resuscitation Certificate or First Aid Certificate (or equivalent).
 
Associate: persons who may or may not hold a SLSA award. Are covered by insurance
 
Life Members: persons who have rendered distinguished or special service as provided by the constitution.
 
The member’s age is how old they are on the 30th September of the current year.

Membership Application Procedure:
 
New members and rejoining members should download the Membership Application Form.
 
A completed application form is required for every member as per the categories described above. The completed and signed forms must be returned together with a payment authority, to the Mermaid Beach Surf Club Office. This can be done in three ways:
 
  1. In person on the nominated sign-on days, with payment by cash, cheque or credit card.
  2. By fax to (07) 55752489 marked Attention Administration Officer
  3. By e-mail with the signed form scanned and attached to the registrar@mermaidslsc.org.au
 
Commissioner for Children, Young People and Child Guardian – Volunteer Blue Card Application:
 
It is a condition of membership that all persons over the age of 18, who are involved in nipper activities, eg Age managers, Officials, Waters safety and all patrolling members must hold a current Blue Card. If you do not have a current card, new or renewal forms can be obtained from the office. The form together with two original identity documents which together show their full name, signature, photo, date of birth and current address must be bought to the club where the club official on duty will sight these original documents and complete and lodge the form with the Commissioner on the member’s behalf. The card will be posted out to the member’s home address. A Blue Card is current for 2 years from the date it is processed.
